Talent Acquisition Module - New User - Creating Interview Feedback Forms Assistance

Issue - Can’t create interview feedback forms the way we would like by creating questions in the question bank, and even if we could, it seems like we will have hundreds of questions in the bank as we have one on one interviews that use unique questions in addition to a standard set of questions that we use for panel interviews.

Example - We would like insert an blank space, or add a line of text in between a set of questions to show on the interview form.

Question - Does anyone create Interview Feedback Forms perhaps a non Eightfold way and are able to attach the form to an interview or being able to use html or other coding to bypass the limitations of the question types provided by the Eightfold system?

Hi @CRatner, Thanks for your question. I hope this is helpful.

In Eightfold interview feedback forms, there isn’t currently a supported way to insert a true “blank space” or a static line of text (like a separator) between questions.

Recommended workaround
The best approach is to use existing question types to simulate this. For example, you can add a dedicated question that acts as a section header or instructional divider. A common method is:

  • Add a Text Box or Text Area question
  • Use the question prompt to display your header or instructions (e.g., “Candidate Experience Feedback”)

This provides a clear visual break while staying within supported functionality.

Additional guidance

  • For open-ended or instructional content, Text Area / Text Box works best due to flexibility and readability
  • Some question types (e.g., rating, checkbox, upload) may not be ideal depending on the workflow—especially in AI Interviewer or screening scenarios

How forms are managed in Eightfold
Interview feedback forms are created and managed in:

  • Admin Console → Feedback & Assessments → Forms Library / Feedback Forms

From there, admins can build forms using supported question types such as:

  • Text, Text Area, Rich Text
  • Checkbox, Multi-choice, Radio, Dropdown
  • Skill Rating, Star Rating, Upload

Attaching forms to interviews

  • Feedback forms are selected during interview scheduling
  • In AI Interviewer workflows, the feedback template is chosen as part of the setup process

About custom HTML or external forms
Eightfold does not support injecting custom HTML, scripts, or external form elements into feedback forms. The builder is limited to native question types.

There is a JSON Editor available for advanced configuration, but it still operates within Eightfold’s supported structure (not custom rendering). For example, you can adjust things like switching a field from text to textarea, but not add non-question elements like spacers or dividers.

Hi Drew,

Thanks so much for all the details.  We have tried using a blank Text Area box question to separate sections of an Interview Feedback Form.  However, the question requires a title that also appears on the Feedback Form which is not ideal either.

This Feedback Form flexibility appears to be a real limitation to the Eightfold product.

Do you know if there is a way to attach a separate file for the interviewer to fill out to be used for interview feedback?

Hi @Cordell, Ahhh. Thanks for sharing. In Eightfold, the intended way to collect interview feedback is to attach an Eightfold feedback form (built in the Forms Library / Feedback Forms) to the interview, rather than attaching a separate file for the interviewer to fill out. Feedback forms are designed to “travel” with the interview schedule/templates and get sent to interviewers right after the interview ends.
